A MESSAGE TO OUR PATIENTS
We are pleased that you have chosen the Lung Care Group for your care. We feel it is important care. We also feel it is important that you are aware of the financial policies of this practice and encourage you to discuss any questions or concerns you may have with us.
We will file a claim with your insurance company on your behalf provided we have a current copy of your insurance card with the correct mailing address. We will make a copy of your insurance card each visit. If your insurance changes between office visits, please notify the office. We file claims promptly and request payment on your account if the insurance company has not paid within 60 days. Co-payment is collected at the time of your appointment.
If you do not have health insurance, we accept Visa and MasterCard as well as personal checks and cash. Payment in full is expected at the time of service unless arrangements have been made with our business office at 317-574-4747. You may contact them to discuss any billing questions or concerns.
REFERRALS
There are many insurance companies that require a preauthorization or referral number be obtained prior to an appointment with a specialist. We ask that you check with your insurance company and, if necessary, obtain the referral from your primary care physician, prior to your appointment. Failure to obtain the appropriate referrals will result in a cancellation of your appointment.
CANCELLATION POLICY
There will be a $50.00 charge posted to your account if you have an appointment and fail to cancel it within 24 hours prior to your appointment time. Insurance companies DO NOT reimburse for cancellation fees. Therefore, you are responsible for paying this fee. To cancel your appointment, please call our office at 317-566-0104 or 800-266-1184.
